Summer Sports Camps Application
All football players participating in summer workouts must
sign and return the following form:
Football Summer Workout Form
Athletic Physicals
Athletic Physicals
All student athletes must have a physical examination before they can participate in fall and spring semester athletics. For more information about physicals, please contact Jim Kurowski - jkurowski@bdhs.org, Melody Hopkins - mhopkins@bdhs.org, or Nurse Kelly Trojan - ktrojan@bdhs.org.
Physicals are required yearly for any student who participates in Band, Drill Team, Cheerleading, and/or Sports. Students will not be allowed to participate until an up to date physical is on file. Please NOTE the changes to this year’s requirement for physicals.
All students participating in extracurricular activities for the
2008-2009 school year must have had an EKG within the last 2 years in addition
to the physical. A link to The form that is to be completed by a licensed
healthcare practitioner is posted below under the TAPPS physical form link.
Parents are free to take their child to the practitioner of their choice for physicals. Please have the physician fill out both the TAPPS physical form and the EKG form.
Thank You!

From the NCAA Clearinghouse website, prospective student-athletes are able to access information needed to understand the Division I and Division II eligibility requirements, register with the Clearinghouse and access individual Clearinghouse records.
In order to participate in athletics and receive athletically-based financial aid, you must register with the NCAA Initial-Eligibility Clearinghouse and meet eligibility standards. If you wish to register on-line with the Clearinghouse, you must have a valid U.S. Social Security Number and a VISA or MasterCard (there is a registration fee of $50.00). If you received a fee waiver for the ACT or SAT exam and are requesting a waiver of the Clearinghouse fee, you may indicate this in Section VI. NOTE that your registration will not be processed until the Clearinghouse receives a waiver confirmation from your high school counselor. If you are seeking a waiver, you should talk to your high school counselor to determine if you are eligible for the waiver. If so, your counselor will need to submit a waiver confirmation for you. High school counselors who have been issued a secure PIN may submit such confirmations through the High School Administrators section of the website.
Important
Note:
Transcripts and test scores must be sent directly to the Clearinghouse.
These documents cannot be faxed or submitted online. There are many reasons
why a student's academic document could be determined to be "unofficial" and
therefore unacceptable for Clearinghouse purposes. If
a document is not received in an official high school envelope or is
received with the student's return address, this document could be marked
as "unofficial." "Official" high
school documents must be received in an envelope easily determined to
be from the school and on high school stationery. Test scores
must either come to the Clearinghouse directly from the testing agencies
(selection of the Clearinghouse can be made using code "9999")
or on an "official" high school transcript, normally using
the testing agencies' labels. Please note that Student Score Reports
and test scores taken from a Student Score Report cannot be used by the
Clearinghouse.
Please contact Mr. Walter Johnson at wjohnson@bdhs.org if you need additonal assistance.
